I am hereby requesting a Functional Behavior Assessment for my son/daughter, (student’s name) to address the ineffectiveness of his/her current behavioral needs. I would like the assessment determine the antecedents to the behavior issues as well as appropriate interventions to teach replacement behaviors. Please consider this letter my consent for the school district/charter school to provide the functional Behavioral Assessment (FBA).
If a meeting is required to pursue the FBA please schedule one as soon as possible.
I expect to be included in the functional behavioral assessment and as an active participant on the team developing the behavior intervention plan. I am requesting that the assessment and classroom observation be provided by a professional with experience in performing FBA's and positive behavior plans. Please provide me with copies of all FBA data and results as soon as they become available to you.
Should this request be declined, please provide me with a Prior Written Notice, as well as an explanation of our rights.
